Job Overview:

Our client is seeking a dedicated Hospice RN to join their after-hours team. This role involves providing exceptional hospice care to patients in their homes during the evening and overnight hours. Responsibilities:

- Collaboratively manage and deliver comprehensive patient care plans during after-hours.

- Provide critical end-of-life care to patients in their homes in Irwin, PA, and surrounding areas.

- Ensure timely completion of documentation and meet critical deadlines for charting.

- Self-manage schedules to ensure efficient use of working hours.

- Utilize technological tools such as tablets and EMR software effectively to enhance patient care delivery.

Schedule and Shift Details:

- Shift options include Monday – Thursday from 5 PM to 8 AM, with flexibility to consider alternate shifts such as Monday – Friday from 5 PM to 1 AM or 12 AM to 8 AM.

Requirements:

- Valid RN license in the State of Pennsylvania.

- Associate’s Degree in Nursing or higher.

- At least one year of professional experience in hospice care, inpatient units, LTC, or oncology.

- Proficiency in using mobile devices and electronic medical records.

- Valid Driver’s License, reliable mode of transportation, and auto insurance.

- Ability to manage travel within the local area.
