Hospice RN Axxess Experience

Join University Home Hospice Inc as a Hospice RN with Axxess Experience, providing compassionate and expert nursing care to patients and families in Wayne, Oakland, and Macomb counties. Responsibilities

• Provide skilled nursing care to hospice patients in their homes across Wayne, Oakland, and Macomb counties.


• Utilize Axxess software to document patient care, update care plans, and communicate effectively with the interdisciplinary team.


• Assess patient conditions, manage symptoms, and collaborate with physicians, social workers, and other healthcare professionals.


• Educate patients and families about hospice care, symptom management, and disease progression.


• Ensure compliance with regulatory and organizational standards related to hospice nursing care.


• Participate in on-call rotations as necessary to support patient needs.

Requirements

• Current RN license in the state of Michigan.


• Experience with Axxess hospice software required.


• Prior hospice or home health nursing experience preferred.


• Strong clinical assessment and communication skills.


• Ability to work independently and manage patient care plans effectively.


• Reliable transportation to travel within Wayne, Oakland, and Macomb counties.
