Job Description - Hospice RN Case Manager

We are currently seeking a RN Case Manager in the Austin area who shares our passion for providing dignified, compassionate end-of-life care. 


Shift Schedule: Monday-Friday

Qualifications: 



  • Must be a Registered Nurse with a valid state license

  • One year of experience working with your RN license required

  • Hospice experience preferred, but not required

  • This role requires a valid driver’s license and a good driving record as a condition of employment. Associates must comply with all state and federal driving regulations

  • Associates in roles requiring driving are responsible for maintaining a valid and active driver’s license. Failure to maintain a valid license may result in reassignment, suspension, or termination in accordance with company policies and applicable laws

  • Reliable transportation and willingness to drive to patient’s location

  • Must be able to lift 50 pounds

  • Must clear a criminal background check, physical and drug screening

  • You may be required to work outside of your regular hours depending on business needs


What you'll be doing: 



  • Develop a detailed Plan of Care for assigned patients ensuring all services needed are identified and met while working closely with the Interdisciplinary Group, patient, family and attending physician

  • Responsible for the coordination of care for the patient and family, providing skilled Hospice and Palliative care for the remainder of patient’s life

  • Provide comprehensive nursing evaluations and assessments

  • Provide field supervision to LVN’s and Home Health Aides assigned to your patients

  • Filed case management

  • Other duties as assigned

Anticipated pay range

$91,000 - $93,000 USD
