Hospice RN Case Manager

You'll coordinate hospice care for patients and families across St. Louis County (excluding city limits). You'll manage your own caseload, develop care plans, conduct assessments, and handle clinical documentation using NetSmart (charting bedside with tablets). Regular collaboration with a supportive clinical team, including physicians, social workers, and hospice aides, will be part of your daily routine.

  Current Missouri RN license (unrestricted).

  Minimum 1–2 years of RN experience required (hospice preferred).

  Valid driver's license, reliable transportation.

  Competitive pay range: $77,962 – $98,006 annually (based on experience).

  Separate On-call positions available to reduce staff nurse on-call load.
