 The Hospice RN Case Manager is responsible for coordinating and managing the care of patients in a hospice setting, providing skilled nursing services and overseeing the implementation of care plans. Primary Responsibilities and Duties:

• Conduct initial and ongoing assessments, including OASIS documentation
• Create and manage individualized care plans in collaboration with an interdisciplinary team
• Coordinate with physicians, therapists, aides, and families to ensure cohesive care
• Supervise and support field staff (RNs, LPNs, Therapists, HHAs)
• Maintain timely, accurate documentation and communicate effectively with care teams
• Conduct comprehensive patient assessments to identify healthcare needs, develop individualized care plans, and update plans as needed.
• Provide skilled nursing care, including pain and symptom management, medication administration, and patient education, in accordance with the care plan and physician orders.
• Communicate effectively with patients, families, physicians, and other members of the healthcare team to update them on patient progress, discuss treatment plans, and address any concerns.
• Monitor patients' physical, emotional, and spiritual well-being, ensuring their comfort and dignity during end-of-life care.
• Ensure compliance with regulatory guidelines, documentation requirements, and facility policies and procedures related to hospice care.
• Provide bereavement support and resources to patients' families and caregivers as needed.
• Participate in team meetings to discuss patient care plans, progress, and any barriers to achieving patient goals.
• Perform all other duties assigned by the Hospice Director or designated supervisor.

Job Qualifications:

• Active RN license (Nevada).
• Minimum 2 years of nursing experience (home health preferred).
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Reliable transportation and a valid driver’s license.
• Current CPR certification.
• Knowledge of regulatory requirements and guidelines governing hospice care and documentation.
• Demonstrated commitment to providing compassionate, patient-centered care in a hospice setting.
• Experience with electronic medical record systems and documentation preferred.

Physical Requirements:

• Ability to lift and transfer patients, as well as carry equipment and supplies weighing up to 50 pounds.
• Capable of prolonged standing, bending, squatting, and walking during home visits.
• Fine motor skills for documentation and administering treatments.
• Ability to respond swiftly and efficiently to emergency situations.
• Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.
• Job requires accepting criticism and dealing calmly and effectively with high-stress situations. 
• The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.
