Position Summary:
The Hospice RN Liaison is a vital member of the clinical outreach team who builds relationships with hospitals, physician offices, skilled nursing facilities, and other healthcare settings to identify patients appropriate for hospice care. This role includes assessing clinical eligibility, educating families and providers on hospice services, and facilitating smooth, compassionate transitions to our hospice program.

Key Responsibilities:

• Serve as the clinical expert in hospice services for referral sources
• Act as an ambassador for      hospice care, advocating for patients and families in various healthcare      settings.
• Perform patient evaluations to determine hospice eligibility and admit to services if appropriate
• Provide education to patients, families, and healthcare professionals regarding hospice philosophy, services, and the admission process
• Collaborate with physicians, case managers, discharge planners, and facility staff to promote timely referrals
• Coordinate with the hospice admission team to ensure seamless transitions
• Attend interdisciplinary team meetings, community events, and outreach initiatives as needed

• Current Texas RN license required
• Minimum 2 years of clinical experience (preferably in hospice, palliative care, or case management)
• 1-2 years’ experience in hospice/palliative marketing strongly preferred.
• Strong interpersonal and communication skills
• Ability to work independently and as part of a team
• Knowledge of hospice regulations and eligibility criteria preferred
• Valid driver’s license and reliable transportation with valid insurance
