JOB SUMMARY:
Our client is a Hospice looking for full time RNs. Will be working with both homecare and facility located patients.
For homecare patients, travel is reimbursed.
SHIFT:
M-F 8a-5p with some potential OT (past 5p), 40 hrs a week.
ESSENTIAL FUNCTIONS
1. Provides nursing care.
1.1 Performs initial and ongoing assessment of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial, and environmental needs, activities of daily living, risk for pathological grief, cultural and spiritual implications, and verbal and non-verbal communication patterns.
1.2 Performs nursing assessments, i.e. Vital signs, pain assessment, breath sounds, etc.
1.3 Triages patient care problems.
1.4 Manages discomfort and provides symptom relief.
1.5 Performs specialized nursing skills related to palliative and end-of-life care.
1.6 Initiates appropriate preventive and rehabilitative nursing procedures.
1.7 Consults with and educates the patient and caregivers (including home health staff) regarding disease process, self-care techniques, end of life care, death and dying, comfort measures, medication use and side effects, process for dealing with ethical issues, etc.
1.8 Performs on-going evaluation of patient/family response to care.
1.9 Assesses the ability of the caregiver to meet the patient's immediate needs upon admission and throughout care.
1.10 Provides psychosocial support to patients and families.
2. Coordinates nursing, personal and homemaker care.
2.1 Coordinates care with team members.
2.2 Informs the physician and other personnel of changes in the patient's needs and outcomes of intervention as needed.
2.3 Coordinates personal care, DME, and obtaining medications and medical supplies.
2.4 Coordinates all patient/family services and prioritizes needs with the members of the IDT.
2.5 Documents in a timely manner patient assessments, patient contacts, physician orders and progress notes that demonstrate progress towards established goals.
2.6 Acts as a liaison between patient and physicians, hospital staff, home health staff, and other care team members to facilitate ongoing clinical care.
2.7 Works with hospital staff, when applicable, ordering DME, meds, etc.
2.8 Evaluates own needs for support (self care and using identified system(s) to meet the need.
2.9 Recognizes and maintains professional boundaries in relationships with patient/family/caregiver.
2.10 Provides preparation and support for patient’s death; pronounces deaths in accordance with state regulation, provides post mortem care, handles body and death event with respect and dignity and in accordance with patient/family/caregiver wishes.
