Hospice Social Worker

Apply now — we're hiring a Hospice Social Worker in Twin Falls.

You’ll work face-to-face with patients and families, supporting them through end-of-life care. Expect a mix of home visits, memory care, long-term care, and wherever else your support is needed. It’s flexible, challenging, meaningful work with a supportive team behind you.

Pay starts at $30/hr and goes up to $36/hr based on experience.

• Manage a caseload of about 24–30 patients


• Handle roughly 4 visits per day, more if there’s a family crisis


• Join a weekly IDT team (Wednesdays or Fridays depending on location)


• Use CareEfficient for charting and documentation


• Provide emotional, psychosocial, and resource-based support for patients and caregivers


• Rotate on-call duty with other social workers (one week at a time with flex time available)

• Bachelor’s or Master’s in Social Work (MSW preferred)


• Driver’s license, reliable car, auto insurance


• Experience in hospice or a similar setting is a plus


• Comfortable working independently, managing your schedule, and entering homes or facilities
