Schedule: Full-time; Monday-Friday 8am-5pm

About the Role:

Hospice of Marion is seeking a Social Worker to support patients and families during their end-of-life journey. This role provides psychosocial support, care coordination, and resource assistance to ensure comfort and dignity for those we serve.

Key Responsibilities:

• Assess patient and family needs related to coping, mental health, financial resources, and caregiver support.


• Provide individual and family counseling, crisis intervention, and bereavement support.


• Collaborate with the Interdisciplinary Group (IDG) to develop and manage patient care plans.


• Assist with financial resources, Medicare/Medicaid, and long-term care planning.


• Maintain accurate electronic medical records (EMR) and participate in IDG meetings.


• Support patients and families with advance directives and memorial planning.


• Educate the community on hospice care.


• Participate in an on-call rotation.

Qualifications:

• Education/Experience: Bachelor’s degree in social work or a related field; one year of clinical experience preferred.

Physical Demands:

• Ability to lift up to 25 pounds frequently and up to 250 pounds with equipment.


• Frequent mobility and possible exposure to communicable diseases and various environments.
