Hospice/skilled home care Intake must have 6mths experience

Description of the role:

We are seeking a Hospice/Skilled Home Care Intake professional with at least 6 months of experience to join our team at University Home Care in Livonia, MI.

Responsibilities:

• Conduct intake assessments for hospice and skilled home care patients


• Collect and review patient information


• Coordinate with healthcare providers and insurance companies


• Ensure compliance with all regulations

Requirements:

• Minimum of 6 months experience in hospice or skilled home care intake


• Proficiency in Medicare, kinnser, access, home care, or home base software


• Excellent communication and organizational skills


• Ability to work independently and as part of a team

Compensation:

$15.00 - $25.00 per hour
