Hospital-Integrated Community Health Worker

Location: Valentine, Nebraska

Position Host Site: Cherry County Hospital and some off-site work within NCDHD district.

Status: Full Time-40hrs/week

Community Health Worker (CHW)

Position Summary

Cherry County Hospital is seeking a compassionate and community-focused Community Health Worker (CHW) to support patient care coordination and chronic disease management across our rural service area. This position works closely with hospital and clinic teams to bridge the gap between healthcare services and the community by helping patients navigate care, access resources, and improve overall health outcomes.

The CHW will play an important role in addressing social determinants of health, promoting healthy lifestyle changes, and strengthening connections between healthcare providers and community services. This position is part of the Rural Health Transformation Initiative.

Essential Duties and Responsibilities

Responsibilities include, but are not limited to:

  • Promote the mission, vision, and values of Cherry County Hospital.
  • Provide support at local WIC and immunization clinics on the western side of the NCDHD district.
  • Attend community events and assist individuals with navigating healthcare systems and understanding insurance benefits.
  • Deliver culturally sensitive health education, basic screenings, and coaching focused on disease prevention and healthy lifestyle habits.
  • Connect clients with community resources and social services to address social determinants of health.
  • Serve as a liaison and advocate between clients and healthcare providers to ensure individual and community health needs are addressed.
  • Document client interactions, assess community needs, and communicate feedback to healthcare teams to improve quality of care.
  • Support coordinated service delivery and improve access to preventive healthcare services.
  • Strengthen partnerships between healthcare organizations and the community.
  • Maintain knowledge of and compliance with Corporate Compliance policies and procedures.
  • Follow Cherry County Hospital personnel policies and procedures.
  • Perform other duties as assigned by supervisor.

Education and Experience

  • High school diploma or GED required.
  • Associate’s or Bachelor’s degree in public health, social services, community health, or a related field preferred.
  • Relevant experience in community outreach, healthcare, or public health is a plus.

Physical Demands and Working Conditions

  • Ability to perform standing, bending, and lifting activities.
  • Repetitive use of hands, arms, and eyes, which may result in strain.

Education and Experience
