Job Description - Housing Case Manager / Navigator

About Grace Community Care & Homes

Grace Community Care & Homes provides community-based housing and support services for adults with special needs, disabilities, and Medicaid members in New Jersey.
We help individuals achieve stability and independence through housing supports, skill-building, and inclusion services funded by NJ.

Position Summary

We’re seeking a compassionate and organized Housing Specialist to assist clients in securing and maintaining safe, affordable housing. The Housing Specialist will coordinate with clients, landlords, and managed care providers to ensure successful tenancy, community integration, and compliance with program requirements.

Key Responsibilities

  • Conduct housing assessments and determine client eligibility for housing supports
  • Assist clients with housing search, applications, and move-in coordination
  • Build relationships with landlords and local housing programs
  • Support clients in maintaining lease compliance and resolving tenancy issues
  • Provide ongoing tenancy-sustaining services (budgeting, life skills, advocacy)
  • Collaborate with MCO housing teams and Grace Community Care’s intake and billing departments
  • Maintain accurate documentation and case notes in Therap/HMIS
  • Participate in regular team meetings and training

Qualifications

  • Experience in housing navigation, case management, or social services required
  • Knowledge of NJ FamilyCare/Medicaid, DDD, or Section 8 preferred
  • Strong communication and organizational skills
  • Valid NJ driver’s license and reliable transportation
  • Ability to travel locally for home visits or landlord meetings

Schedule & Compensation

  • Schedule: Monday–Friday, 10:00 AM – 6:00 PM
  • Pay: $21-$23/ an hour
