POSITION SUMMARY

Prestige Healthcare Resources, a leading mental health agency in Washington, D.C., is seeking a dedicated Housing Liaison to support our mission of providing comprehensive mental health services. The Housing Liaison will play a critical role in assisting our consumers in accessing and maintaining stable housing, which is a crucial component of overall mental health and well-being.

RESPONSIBILITIES:

  • Serve as the primary point of contact between consumers, housing authorities, landlords, and other housing providers.
  • Assist consumers in understanding their housing rights and responsibilities to ensure they secure and maintain housing.
  • Facilitate access to housing resources, including applications for housing programs and services.
  • Advocate for consumers with housing providers and external agencies to address and resolve housing issues.
  • Coordinate with clinical teams to integrate housing solutions into broader mental health treatment plans.
  • Conduct SPDAT Assessments with Consumers
  • Maintain accurate and detailed records of interactions, interventions, and outcomes.
  • Develop and maintain strong relationships with community resources and partners to enhance service provision.
  • Conduct regular follow-ups with consumers to ensure housing stability and address any emerging needs.

QUALIFICATIONS

  • High Schook Diploma or GED, or a related field required.
  • Minimum of 1 year of experience in housing services, case management, MHRS program or a related field.
  • Strong understanding of the housing systems and resources available in Washington, D.C.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Strong advocacy and negotiation skills.
  • Ability to work independently and as part of a team.
  • Proficiency in case management software and standard office software (Credible system).
