Job Description - Housing Program Case Manager-Youth

POSITION: Housing Program Case Manager

REPORTS TO: Housing Program Director 

SALARY: As per agency salary schedule FLSA: Non-Exempt

HYBRID WORK: Eligible

JOB SUMMARY: The Housing Case Manager provides support to the Housing Director for 

implementation of program funding with a focus on youth ages 18-24. Through the direction of the Housing Director, Case Managers will assess each individual case to identify client needs and coordinate rental assistance resources and services appropriately. They work closely with the Housing Director to gain increasing knowledge of available housing stock, familiarity with common barriers facing renters and landlords to help individuals and families secure stable and affordable housing. Travel and home visits are required.

ESSENTIAL DUTIES:

  1. Understand and adhere to program rules and restrictions as required by our Grantors.
  2. Complete certification requirements for rental inspections.
  3. Assess and counsel individuals and families who are seeking housing services to determine eligibility.
  4. Screen and process all applications for housing program services.
  5. Maintain and document case management activity/correspondence databases 
  6. Assist clients with the search for available rental properties and negotiate terms on behalf of the client with landlords, property owners and property management companies.
  7. Conduct rental property inspections to ensure the safety of our clients as required by our Grantors.
  8. Create an active Youth Action Group.
  9. Provide transportation for clients if a barrier is identified for housing related activities such as to view properties, apply for rental properties, or to attend appointments at a local housing agency.
  10. Provide case management supportive service to clients working toward self-sufficiency weekly, bi-weekly or at a minimum of monthly as needed.
  11. Ensure clients understand their rights and responsibilities as tenants.
  12. Act as an information source for agency housing programs, collaborate with Outreach Staff to provide program referral services to qualifying individuals and families.
  13. Promote and participate during the Department of Housing and Urban Development required Annual Point-In-Time Count held every January/July.
  14. Attend mandated trainings and meetings.
  15. Attend all Upper Des Moines Region Coordinated Entry Pull Meetings and execute meetings for the Region in the absence of the Housing Director.
  16. Collaborate with government agencies, community organizations, and property owners.

OTHER DUTIES AND RESPONSIBILITIES:

  1. Input, access and retrieve information from computer system.
  2. Serve as back up voting member for the Designated Lead Agency if position is open.
  3. Assist in facilitating Coordinated Entry Regional Meetings.
  4. Perform other duties as assigned.

KNOWLEDGE, SKILLS and ABILITIES:

1. High school diploma/GED required.

2. Degree in human services, social work or other related experience.

3. Experience with housing issues is a plus.

4. Working knowledge of social service agencies and community services.

5. Good communication, interpersonal and listening skills.

6. Strong analytical, mediating and negotiation skills.

7. Excellent organizational and case management skills.

8. Sensitive to different cultures and socioeconomic conditions.

9. Ability to perform medium heavy work, exerting up to 50 pounds of

force on occasion, up to twenty pounds frequently, and up to 5-10 pounds constantly, performing such activities as stooping, kneeling, crouching, reaching, standing, walking, pushing, pulling, lifting, grasping, including some duties requiring repetitive motion for limited amounts of time.
