Housing Supports Program Case Manager

The Housing Supports Program (HSP) offers a supportive housing service aimed at helping individuals secure safe and stable housing, as well as providing assistance in maintaining a stable living environment. Case managers deliver the necessary support within each participant's home and the wider community. HSP renders outreach and case management services to participants, facilitating access to, acquisition of, and effective utilization of resources tailored to their individual needs. Additionally, HSP provides outreach and case management services focused on fostering the development and execution of independent living skills.

Salary: $19.00 Hourly

Responsibilities:

• Provide supervision and support to clients, including:
• Helping clients identify their housing-related needs and setting goals to address those needs
• Assisting with transportation, recreational activities, and medical or psychiatric appointments
• Supporting clients in learning and implementing daily living skills to enhance their independence
• Delivering case management services to help clients sustain their benefits and housing
• Consult with clinical staff regarding client concerns and requirements
• Conduct therapeutic rehabilitative skill development
• Contribute to intake assessment, needs assessments and housing support plans
• Review support plans collaboratively with clients

• Bachelor’s Degree in a Behavioral Health Science
