Job Description - HS Supervisor

Description

  

DUTIES AND RESPONSIBILITIES:

Demonstrates Competency in the Following Areas:

  1. Complies with all applicable      company Policies and Procedures.
  2. Serve      as a resource for technical questions, applications and problem-solving      regarding Community Care Program and In-Home Client Servicing as needed. 
  3. Maintains      knowledge of the various methods of In-Home Care. Ensures client and      employee safety, assesses possible risks, and constantly works on its      prevention.
  4. Ensures that client and employee      records are complete and current (within a week); timely documents in case      records client / employee needs, concerns, activities, and issues related      to client services in the client’s file.
  5. Learns, masters, and effectively / professionally uses      Axis Care program, ensuring daily updates of contained information.
  6. Monitors      receipts of the community Care In-Home Services as stated in the Plan of      Care.
  7. Provides input to the case manager      on the services that are needed for each client as a result of conferences      with the homecare aider or in-home visits.
  8. Manages      and supervises the participants’ needs according to the plan of care of      the participant.
  9. Oversees and manages the case      notes, care plans, schedules, authorizations, etc. for all participants,      as well the work refusal forms, FMLA, insurance applications, etc. for the      branch employees.
  10. Plans and prepares contact and      quarterly conferences with Home Care Aides.
  11. Create,      submit, and track the Critical Event Report (CRE) in the eCCPIS system,      creating office notes.
  12. In      on-call and available whenever service is being provided.
  13. Personally,      substitutes HCA if necessary.
  14. Assists      with customer service and satisfaction. 
  15. Maintains      professional, friendly, courteous, caring relationship/atmosphere with all      staff members.
  16. Consideration      of the schedule review and revision of the payroll.
  17. Attends      all supervisor in-service training.
  18. Submits      required documentation for new hires and on-going management of employees,      including updates of case records.
  19. Makes      in-home visits, as necessary, to provide hands-on training and assistance.
  20. Works      in team to assure accomplishment if the company’s goal.
  21. Performs other duties as assigned.

Completes annual education requirements.

Maintains regulatory requirements.

Reports to work on time and as scheduled and completes work within the designated time.

Uses the punch time system correctly.

Maintains client confidentiality at all times.

Cooperates with other staff members and different offices.

Works in the atmosphere of joint effort, solidarity, and support.

Attends annual review and performs in-services.

Stays current with industry standards by reading/attending educational presentations.

Represents the company in a positive and professional manner in the community.

Actively participates in performance improvement and continuous quality improvement (CQI) activities.

Attends management meetings, as appropriate.

Ensures compliance with policies and procedures regarding operations, fire, safety, and infection control.

Complies with all organizational policies regarding ethical business practices.

Communicates the mission, ethics, and goals of the company.
