Job Description - Human Resources Manager- Home Health/Home Care



The Human Resources Manager is responsible for overseeing all human resources functions for CareSense Home Care, with a strong focus on workforce compliance, recruitment, onboarding, employee relations, retention, and personnel file management. This position helps ensure that all office and field staff are hired, onboarded, trained, and maintained in accordance with CareSense policies and applicable federal, state, payer, and accreditation requirements.


The Human Resources Manager plays a key role in supporting survey readiness, personnel file audits, corrective action follow-up, and ongoing compliance for home care operations.


Essential Duties and Responsibilities




  • Oversee day-to-day HR operations for CareSense Home Care.




  • Manage and support recruitment, interviewing, hiring, and onboarding of caregivers, home health aides, direct care workers, administrative staff, and other agency personnel.




  • Ensure all new hire and ongoing employee documentation is complete, accurate, current, and properly maintained in personnel files.




  • Monitor personnel files for compliance with agency standards and regulatory requirements, including but not limited to:






      • Employment forms




      • I-9 and W-4




      • Clearances and background checks




      • Health screenings and TB documentation




      • Licenses, certifications, and CPR, if applicable




      • Orientation, training, and annual in-service records




      • Competencies and other job-required documents








  • Track expiration dates for licenses, certifications, clearances, health documents, and other required items; ensure timely follow-up and renewal.




  • Conduct routine personnel file audits and assist with monthly, quarterly, or corrective-action-based HR compliance reviews.




  • Maintain audit tools, tracking logs, and follow-up documentation related to missing, expired, or deficient employee records.




  • Partner with branch leadership, clinical leadership, and operations staff to ensure staffing files remain compliant and survey-ready.




  • Assist in developing, updating, and implementing HR policies, procedures, forms, and employee handbook materials.




  • Provide guidance to supervisors and managers regarding employee relations, attendance, performance concerns, corrective action, coaching, and terminations.




  • Investigate employee complaints, workplace concerns, and HR-related issues promptly, professionally, and confidentially.




  • Support employee retention efforts, engagement activities, and staff accountability initiatives.




  • Coordinate and monitor orientation and annual education requirements in collaboration with agency leadership.




  • Ensure required acknowledgments, training sign-off forms, and performance-related documentation are completed and filed timely.




  • Assist with unemployment claims, verification requests, worker-related documentation, and other employment matters as needed.




  • Maintain confidentiality and security of employee records and sensitive HR information.




  • Prepare HR reports, audit summaries, corrective action updates, and other documentation requested by leadership or surveyors.




  • Support agency readiness for payer audits, licensure reviews, accreditation surveys, and internal compliance monitoring.




  • Perform other duties as assigned to support CareSense Home Care operations.




Compliance and Regulatory Responsibilities




  • Help ensure personnel files are maintained in accordance with applicable home care regulatory and agency requirements.




  • Monitor employee documentation to support compliance with Pennsylvania home care expectations, payer participation standards, and internal agency policies.




  • Assist leadership with preparation for surveys, audits, credentialing reviews, and corrective action plan follow-up.




  • Maintain organized and retrievable HR records for review by authorized leadership, auditors, and surveyors.




  • Identify file deficiencies or compliance risks and escalate concerns to leadership promptly.




  • Support implementation of corrective actions related to personnel records, onboarding, training, and HR process gaps.




  • Promote consistent documentation practices across branches and departments.




Supervisory Responsibilities




  • May supervise HR staff, recruiters, onboarding coordinators, or administrative team members.




  • Provides HR guidance and support to branch leadership and department managers.




Qualifications




  • Bachelor’s degree in Human Resources, Business Administration, Healthcare Administration, or related field preferred.




  • Minimum of 3 years of human resources experience required.




  • Prior experience in home care, home health, healthcare, or other regulated healthcare setting strongly preferred.




  • Experience with employee onboarding, personnel file compliance, training records, and employee relations required.




  • Knowledge of HR practices, employment laws, and workforce documentation requirements.




  • Experience supporting audits, surveys, or accreditation readiness preferred.




  • Strong organizational skills and ability to manage multiple deadlines and priorities.




Knowledge, Skills, and Abilities




  • Strong knowledge of HR operations, employee relations, and personnel documentation practices.




  • Ability to maintain confidentiality and handle sensitive employee matters appropriately.




  • Strong attention to detail and ability to identify missing, incomplete, or expired documentation.




  • Ability to interpret and apply agency policies and compliance expectations.




  • Effective written and verbal communication skills.




  • Strong follow-up, problem-solving, and organizational skills.




  • Proficiency in Microsoft Office and HR, scheduling, or personnel record systems.




  • Ability to work independently while collaborating effectively with leadership and branch staff.
