Job Description - I/DD Oregon Needs Assessor

Center for Human Development, Inc. is seeking a full time I/DD Certified Functional Oregon Needs Assessor. The Needs Assessor will work with the Intellectual/Developmental Disabilities team to conduct functional needs assessment to determine if an individual eligible for Medicaid Funded Services, meets the required Level of Care, establish service level and/or service payment. Complete and submit assessments in ODDS’ electronic payment and reporting system.


    


The Requirements:


Must possess a valid driver’s license and pass all criminal history check requirements and drug screenings.


 


An assessor and supervisor must have knowledge of the public service system for developmental disabilities services in Oregon and at least:


(a) A bachelor's degree in behavioral science, social science, or a closely related field;


(b) A bachelor’s degree in any field and one year of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing;


(c) An associate’s degree in a behavioral science, social science, or a closely related field and two years of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing; or


(d) Three years of human services related experience, such as work providing assistance to people and groups with issues, such as economical disadvantages, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, or housing.



Essential Functions:



  • Conduct the Oregon Needs Assessment (ONA)

  • Complete all required trainings to maintain certification, as directed by ODDS virtually and in person

  • Conduct and complete assessments as directed of ODDS and OAR’s

  • Conduct file reviews, face to face observations and interviews with the individuals, families and/or other supporters

  • Collaborate with Service Coordinators to ensure accuracy of assessments

  • Submit assessments in a timely manner and within established timelines

  • Monitor due dates and deadlines for upcoming assessments needed

  • Maintain accurate and timely documentation and progress notes for assessments completed

  • Interact with individuals using Person Centered Practices

  • Attend regularly scheduled team meetings, assessor call ins and other meetings as needed virtually and in person



  • Have the ability to work effectively as a member of a team.
