About the Role:

We are seeking a dedicated VRS and HSS Case Manager to join our team at Kesma Flame Lily, LLC in Minneapolis, MN. This role offers the opportunity to make a meaningful impact in the lives of our clients and contribute to the growth of our company.

Responsibilities:

Provide case management services to individuals utilizing (Independent Living Skills) and Housing Sustainability Services (HSS)

Assess client needs and develop individualized service plans

Coordinate with healthcare providers, social services, and other relevant agencies

Advocate for clients to ensure access to necessary resources and support

Document client interactions and progress in case management software

Requirements:

Bachelor's degree in social work, counseling, or related field

Experience working with individuals in housing sustainability, and independent living skills

Knowledge of ARMHS and HSS programs and services

Strong communication and interpersonal skills

Ability to work independently and as part of a team

Compensation: $19.00 - $24.00 per hour
