Job Description:

Phoenix Home Care & Hospice is hiring Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) to provide 1:1 skilled nursing care in the homes of our clients in Mt. Vernon, MO. Job Responsibilities:

• Follow individualized plans of care developed in coordination with physicians
• Monitor, document, and report patient conditions using electronic charting
• Educate and support clients and their families
• Collaborate with the care team and attend staff meetings as needed

Qualifications:

• Active State of Missouri or Compact Nursing License (LPN or RN)
• Strong communication and critical thinking skills
• Professional, positive, and compassionate attitude
• Ability to work independently and manage time effectively
