Job Description:

Phoenix Home Care & Hospice is seeking compassionate LPNs and RNs to provide one-on-one care in the homes of our clients. We prioritize your professional satisfaction and personal well-being, so you can focus on delivering the highest quality care.

Responsibilities:

• Follow an individualized plan of care as directed by the physician
• Monitor and document patient condition and care provided
• Provide education and support to clients and families
• Complete electronic charting and documentation
• Participate in training sessions and staff meetings

Requirements:

• Active LPN or RN license (State of Missouri or compact)
• Strong communication and problem-solving skills
• Professional, positive, and flexible attitude
