Job Description - In-Home Community Specialist


Earn $600–$700 per day while making a meaningful difference.

WestBridge, Inc. is seeking compassionate, dedicated professionals to join our Community Integration Program as an In-Home Community Specialist. This unique live-in position offers the opportunity to build meaningful relationships while supporting adults in their recovery from mental illness, substance use disorders, and co-occurring conditions.

Compensation: Live-in support shifts are paid $600–$700 per day, based on experience, qualifications, and participant assignment.

As an In-Home Community Specialist, you will become an integral part of a transdisciplinary treatment team, providing one-on-one support in a participant's home. Through mentorship, guidance, and practical assistance with daily living, you will help participants build independence, develop life skills, and achieve the goals outlined in their individualized treatment plans.

Responsibilities:

Working on a 3 week rotating schedule:

Week 1: Live-in 24/7 support, residing in the participant’s home and supporting them with Activities of Daily Living (ADLs).

Week 2: Not actively working, attending one-to-two team meetings to remain aware of treatment progress.

Week 3: Standby support for current live-in staff in case of illness or emergency that inhibits their ability to provide prescribed services.

  • Changes to the above rotation may take place based on need (ie. Participant travel that takes place over the course of more than one week).
  • Support alongside participants including but not limited to preparing meals, laundry, cleaning, and wellness activities to help maintain a healthy living environment.
  • Provide connection to community resources including but not limited to attending 12-step meetings or recovery-related activities.
  • Providing exposure to positive activities in the community including but not limited to concerts, sporting events, personal and family vacations.
  • Modeling healthy and therapeutic boundaries.
  • Provide the participant access to their medications and monitor the participants’ self-administration of medications.
  • Attend regular family clinical update meetings modeling effective communication techniques and supporting participant in being an active member of their treatment team.
  • Attend regular team meetings to collaborate with clinical staff on participant treatment goal progress, providing updates and insights from daily observation.
  • Provide support services to participants outside of New Hampshire in accordance with WestBridge’s Out of State travel policy in conjunction with one clinical staff member.
  • Accessible by cell phone at all times during shift hours.
  • Participates in open, direct, solution-focused, communication with participants and team members.
  • Represents WestBridge in a professional and courteous manner, including: wearing appropriate attire reflective of the assigned activities.
  • Attend regular supervision with Clinical Manager of Community Integration Program.
  • Customer Service Skills: a solution-focus attitude, being responsive and flexible.
  • Completes documentation (ie; Electronic Medical Record, electronic timecard, expense reports, etc.) in a timely manner and in accordance with company policy.
  • Maintains compliance with local, state and federal regulations, including but not limited to HIPAA and 42 CFR part 2.
  • Maintains professional boundaries at all times, adhering to WestBridge Code of Ethics and Employee Handbook.
  • Completes ongoing trainings, as approved by supervisor, and engages in supervision to gain a better understanding of mental health and substance use disorders and recovery (i.e. online or external training, as well as individual and group supervisions).
  • Ability to work across both programs (Residential and Community Integration Program) with proper guidance and training approved by Clinical Manager of Community Integration Program.
  • Adhere to Attendance Policy surrounding scheduled/unscheduled time off.
  • Perform other duties as assigned.

Requirements

  • Knowledge obtained through completion or in pursuit of an Associate/Bachelor degree curriculum in appropriate discipline from accredited institution, OR equivalent combination of education (example: Peer Support Specialist Certification or Certified Recovery Support Worker Certification) and experience.
  • Remain in good standing and maintain Peer Support Specialist Certification or Certified Recovery Support Worker Certification.
  • Knowledge and experience in the treatment of mental illness and substance use disorders preferred.
  • Knowledge of state and federal rules and regulations governing confidentiality.
  • Knowledge of Self-Help etiquette, traditions and culture preferred.
  • Skill in working and communicating with others.
  • Demonstrates a high standard of professional conduct, professional boundaries and ethical behavior that will enhance the quality of care and encourage positive interactions among staff.
  • Must maintain neat and safe functioning of personal vehicles used for work.
  • Valid drivers’ license and appropriate vehicle insurance coverage of: 100,000 bodily injury per person/300,000 bodily injury per accident/100,000 property damage.
  • Must be 21 years or older in order to operate motor vehicle for WestBridge.
  • Ability to access email from home.
  • Regular and reliable attendance is required.
  • Attendance at our annual Weekend of Hope event is required for all WestBridge staff, regardless of position or schedule
  • Pre-Employment Drug Testing Requirement.
  • Satisfactory Criminal Background Screening results (State and Federal).
  • Satisfactory Motor Vehicle Records results.
