Your Responsibilities:

• Conduct in-home PT evaluations and craft custom treatment plans
  
• Educate patients and caregivers on exercises, safety, and mobility
  
• Work with interdisciplinary teams to ensure coordinated care
  
• Document patient progress and update care strategies as needed
  

Requirements:

• MN Physical Therapy license
  
• Driver’s license and reliable transportation
  
• Home care or rehab experience preferred
  
• Passion for serving patients in rural Minnesota
