Inpatient Licensed Practical Nurse - Rehab Nursing, Dayshift

Job Summary:

Administers nursing care for assigned patient population under the direct supervision of a Registered Nurse. Provides leadership and collaborates with the healthcare team to promote a world class environment.

1. Participates and assists in data collection in a systematic and organized manner to provide high quality and safe outcomes.
2. Promotes high quality patient care by: a. performing or assisting with the patient's activities of daily living and utilizing the Relationship Based Care model. b. ensuring a clean and safe environment c. supporting the physician order process d. maintaining the medical record
3. Assists the healthcare team by collecting, reporting, documenting, and delivering care to support high quality and safe outcomes. Maintains high satisfaction levels by providing care with considerationof the individual needs of the patient/family and their age-specific/developmental needs.
4. Collaborates with other team members to evaluate the plan of care with the patient and family/support system to maintain world class levels of patient satisfaction and outcomes.
5. Contributes to a high performance work environment by seeking educational resources that enhance and maintain current knowledge and skills.

Secondary Job Duties That May be Reassigned:

1. Performs other duties as assigned or requested

• Education: Graduate from an accredited school of practical nursing
• Licensure: Practical Nurse license from the Indiana State Board of Nursing. Some area may require a current and valid driver’s license.
• Experience: Graduate of LPN program
