Intake Case Manager

Description

The Intake Case Manager plays a critical role in welcoming and supporting individuals entering substance use disorder and co-occurring residential treatment at Fellowship House. This position emphasizes the importance of first impressions, community engagement, and client-centered support throughout the intake and early treatment process.

The Case Manager collaborates closely with the clinical team to fulfill the mission and service goals of Fellowship House and is responsible for guiding clients from their initial point of contact through the early stages of treatment, while also fostering strong partnerships with community organizations.

-Room inspections

• Conduct initial intake and orientation for clients entering medium-intensity residential program.
• Provide compassionate, customer-service-focused support to clients and their families during the admissions process.
• Coordinate with treatment staff to ensure seamless transitions into care.
• Collect and document necessary intake information, including GPRA (Government Performance and Results Act) reporting and other required assessments.
• Act as a liaison between Fellowship House and external agencies to facilitate referrals, service access, and resource sharing.
• Build and maintain relationships with community partners to expand support options for clients (e.g., housing, healthcare, employment).
• Represent Fellowship House at community events, meetings, and outreach initiatives.
• Provide individualized case management to clients across pretreatment and intake.
• Assist clients with connecting to essential resources such as medical care, clothing, transportation, and benefits.
• Lead or co-facilitate support groups..
• Offer coaching and reinforcement of daily living skills and recovery-focused behaviors.
• Conduct room inspections and support medication management protocols as needed.
• Administer ongoing alcohol and drug testing.
• Maintain accurate, timely records of all client interactions and services.
• Complete billing and reporting documentation per agency and funding requirements.
• Participate in case reviews, staff meetings, and interdisciplinary coordination related to client care.
• Transport clients to essential appointments and community-based services as needed
• Perform similar other duties assigned by the Intake, Family, and Placement Coordinator, other onsite coordinators, Clinical Director, and CEO

Work schedule specific to this employee:

Holiday coverage in rotation with other staff;

Extended hours when needed to provide continue direct care service

Regular Schedule: Monday-Friday 8:00 AM- 4:00 PM

Requirements

• Bachelor's degree in a behavioral health-related field preferred or high school diploma/GED with ADC or CRSS certification.
• Valid driver’s license and good driving record.
• CPR, First Aid, and Medication Assistance Certification (may be obtained through Fellowship House).
• Strong knowledge of addiction, co-occurring disorders, and recovery principles.
• Familiarity with Recovery-Oriented Systems of Care, NIDA’s Principles of Effective Treatment, and local social service resources.
• Commitment to ongoing professional development and client-centered service delivery.
