EXPERIENCE / MINIMUM QUALIFICATIONS
Bachelor's degree in Social Work from an accredited college or university Possess an LLBSW with the State of Michigan, LBSW preferred.
Ability to work closely and communicate effectively with persons with mental and/or physical limitations.
Ability to proceed in carrying out oral and/or written directions given by team members or individual/family.
Must possess a valid driver’s license in the State of Michigan and proof of insurance.
Schedule:
Full-time, Monday–Friday schedule (40 hours per week), typically 8:30 AM–5:00 PM. Some schedule flexibility may be available at the discretion of the supervisor with appropriate advance notice.
Essential Key Functions:
As an Intake Community Screener, you will play a pivotal role in promoting recovery for individuals and families served. You will actively engage with individuals and families seeking mental health services, offering a welcoming and supportive intake experience. You will have the role of greeting individuals and families at community partnerships (which may include hospitals, shelters, SUD treatment facilities among other locations) , providing information about available services, and instilling a sense of hope. Greetings may also take place virtually or in an EM office. Your responsibilities will encompass community inclusion activities, advocacy, education, and various direct services aimed at enhancing the recovery journey of our individuals and families served.
Promotes and participates in activities that enhance individual and family recovery such as community inclusion activities, advocacy and anti-stigma, education opportunities, etc.
Enhances engagement and outreach of new individuals and families by providing a welcoming, supportive intake experience and offering smooth transition from an individual’s first access point for services to intake appointment and to ongoing program services.
Direct services primarily include outreach and engagement in acute care settings, hospitals or in the community. Virtual and office-based services may be assigned as needed. It may also include linkage to community supports, coordination with community partners, in-home services, officebased services, and advocacy.
Attends team meetings and receives assignments from supervisor, team leader or designee. Assists team and staff in identifying program environments that are conducive to recovery; lending their unique insight into mental illness or dual diagnosis and what makes recovery possible.
Educates new individuals and families on EM service array and person-centered planning process, often in the hospital and community settings. Promotes individual and family decision-making and assists individuals and families in determining goals and objectives that will help reach recovery.
Assists individuals and families in obtaining services that suit their individual recovery needs.
Completes the intake pre-session process for individuals and families wishing to obtain services. Pre-session process includes a series of intake forms, screening tools, and may include the Request for Services document as needed.
Reports and documents service activity according to Easterseals MORC’s Operations Manual, Medicaid Provider Manual and Michigan Mental Health Code.
Demonstrates knowledge of principles of trauma informed care.
Willingness to support and assist individuals and families with trauma related issues.
