Job Title: Intake Coordinator

Job Summary:

We are seeking a dedicated and organized Intake Coordinator to join our home-based primary and palliative care team. The ideal candidate will be responsible for managing the intake process for new patients, ensuring a smooth transition into our care services. This role requires excellent communication skills, attention to detail, and a compassionate approach to patient care. 

This position is an in-office position working Monday-Friday 8am-5pm or 9am-6pm.

Key Responsibilities:

• Manage the intake process for new patients, including gathering necessary information and documentation.
• Coordinate with healthcare providers, patients, and families to facilitate seamless communication and care transitions.
• Maintain accurate and up-to-date patient records in compliance with privacy regulations.
• Assist in scheduling appointments and coordinating care plans with interdisciplinary teams.
• Monitor and follow up on patient referrals and ensure timely responses.

Requirements

Skills and Qualifications:

• Previous experience in a healthcare setting, particularly in intake or patient coordination, is a plus.
• Strong organizational skills and attention to detail.
• Excellent verbal and written communication skills.
• Ability to work independently and as part of a team.
• Proficient in using electronic health record (EHR) systems and other relevant software.
• Compassionate and patient-centered approach to care.
