Job Description - Intake Coordinator

Paradigm Health is expanding rapidly throughout Indiana and we are opening several new positions. Due to our continued growth, we are adding an Intake Coordinator.

Work Location:

Indianapolis Office

Kokomo Office

Hybrid Option

Work Hours:

Monday-Friday 8 AM-5 PM

Requirements

Functions:

  • Support the business development team in obtaining required information & documentation for incoming referrals.
  • Monitor faxes and other intake sources for incoming referrals and other referral related information. (Forcura, Epic, Allscripts, Cerner, etc.)
  • Enter patients in EMR system (WellSky), create cases in intake software (Forcura) and ensure information flows over. Enter new physicians, and referral sources at necessary.
  • Initiate communication with business development team for all incoming referrals.
  • Demonstrate initiative in requesting additional documentation as needed to support eligibility, proper coding and accurate billing in accordance with hospice Medicare regulations.

Experience:

  • Previous medical office/administrative experience required. 
  • Experience with Hospice referral process including insurance verification is preferred but not necessary.
  • Excellent oral and written communication skills with accurate data entry skills and a high comfort level with computers required.
  • Candidates must be able to multi-task and carry a hectic workload at times.
  • Active driver's license and reliable vehicle is required.
