Intake Coordinator

Manages intakes and discharges while working in collaboration with the Clinic al team and insurance 

company to authorize medical necessity for continuation of treatment at a residential level. 

Job Responsibilities 

• Monitors adherence to the hospital's utilization review plan to ensure the effective and efficient use of residential services and appropriateness of residential admissions per Community Care Licensing regulation. 

• Establish networking opportunities with referral resources. 

• Conduct virtual and in person presentation for marketing purposes for programming provide education. 

• Establishes and maintains efficient methods of ensuring the medical necessity and appropriateness of residential admissions. 

• Performs concurrent reviews for Residents to ensure that extended stays are medically warranted and documented in the Resident's medical record. 

• Coordinates and organizes activities of the Utilization Review Department using length of stay as a guideline. 

• Calculates length of stay and continued-stay days for Residents. 

• Refers to Physician, Therapist, Nursing Team and Resident if there is a continuation of treatment and stay. 

• Collaborate with case managers/liaisons to ensure continuity of care of the patient’s treatment plan. 

• Works with patients to coordinate aftercare/ post discharge appointments, resources, and linkage. 

• In person meeting and interviewing potential patients in a hospital setting, as necessary. 

• Assists residents and families in preparing for returning home or to the community successfully. 

• Ensures that services are individualized emphasizing resident needs, abilities, strengths, and preferences. 

• Manages resident crises in a calm and professional manner. 

• Keeps supervisory staff aware of issues related to residents and/or families. 

• Submits clear and concise progress notes and correspondence related to resident care. 

• Maintains accurate records and organized resident charts. 

• Prepares periodic reports concerning resident care. 

• Follows facility safety guidelines including proper lifting technique and Universal Precautions. 

• Adheres to HIPAA Guidelines. 

• Provides coverage in emergency services as needed. 

• Perform other related duties, as necessary. 

Requirements and Qualifications 

• Minimum Education: Bachelor's degree in psychology or Sociology  

• Minimum Experience: One (1) year experience working in a mental health setting or social service agency providing services to mentally ill clients. 

• A Clear criminal record review and fingerprint background check 

• Successful completion of a Negative Drug Test 

• Hiring is contingent upon a full background screening in accordance with California licensing regulations; DOJ/FBI clearance, pre-employment physical and TB clearance. DMV printout and Basic First Aid certification is  prior to employment. 

• Healthcare Provider Basic Life Support Certification 

• Must have an active Driver's License 

• Must provide proof of vehicle insurance coverage of a minimum of $30,000 liability
