Job Description - Intake Coordinator

Azleway is seeking a qualified candidate to serve as the Intake Coordinator for the Humble office. This position reports directly to the regional director and is responsible for coordinating efforts to place children within Azleway’s Foster Home system.


 


Responsibilities and Duties
Duties of the Intake Coordinator include:


 



  • Maintain timely correspondence with CPU’s, SSCC intake staff, CPS Case Workers, JPD Officers, and Parent regarding possible Private Placements. The Intake Coordinator is responsible for maintaining a positive PR relationship with all stakeholders that they interact with.

  • Maintaining and monitor DFPS vacancy list and ensures that Child Placement Management Staff (CPMS) are aware of changes and updates.

  • Data collection for placements to ensure all required documentation is present. Further data collection will be required for PQI purposes.

  • Correspond with foster parents relating to availability for placements and coordination of placements

  • Collect data and information relating to Admission Assessments, and Preliminary Service Plans

  • Monitor required intake information for accuracy and ensure needed information is in case files.

  • Development, upkeep, and coordination of children’s files and foster parent files 

  • Maintain all documentation as required by Minimum Standards for Child-Placing Agencies, Level of Care standards, contract conditions, and Azleway policies.

  • Routinely staffing development, monitoring and training activities with administration

  • Attending weekly meeting to assess and discuss foster home vacancies.

  • Keep agency administration informed of schedules, serious incidents, and problems as they arise.

  • Complete other duties as assigned.


 


Preferred Qualifications and Skills


 



  • A master's degree in a human services field from an accredited college or university, OR

  • A master's degree from an accredited college or university and one year of relevant experience, OR

  • A bachelor's degree in a human services field from an accredited college or university and one year of relevant experience, OR,

  • A bachelor's degree from an accredited college or university and two years of relevant experience.


 


Must meet all applicable Texas Department of Family and Protective Services licensing requirements, including passing mandatory drug screening, background checks, and finger printing.
