Title: Intake Coordinator

Department: Vital Bridges

Shift: FT/ 8:30AM - 5:00PM CST / M-F

Location: 4121 W Lake St

Heartland Alliance Health’s (HAH) mission is to transform healthcare for the most vulnerable – particularly people experiencing homelessness, mental illness or addictions, or struggling with multiple chronic illnesses – improving health for all and the well-being of our community.

Summary: Intake Coordinator is responsible for the intake and processing of food and nutrition applications, maintaining client records and program files and utilizing participant database(s) to produce correct and adequate reports. Individual accountabilities and work volume will be established through the development of annual Success Objectives, within the framework outlined below.

Essential Functions:

Duties & Responsibilities

  • Reviews Vital Bridges applications submitted electronically, faxed, mailed or in-person, for completion and accuracy.
  • Approves or denies applications according to program eligibility requirements
  • Interfaces with applicants or applicant’s representatives regarding program enrollment status
  • Compiles and enters participant data into program databases.
  • Tracks and reports enrollments and program statistical data in excel documents or other tracking platforms
  • Coordinates annual participant data notification updates.
  • Develops and implements processes to maintain program documents in an organized and compliant matter
  • Compiles documents for program monitoring and planning
  • Participates in regular program coordination meetings and other staff meetings and trainings, as indicated.
  • Engages in other duties as assigned

Education and Experience:

  • High School Diploma or equivalent
  • Minimum one year experience working in social services or related field.
  • Bilingual Spanish speaking strongly preferred.

Knowledge/Skills/Abilities (K/S/A)

  • Demonstrated ability to utilize universal and systematic methods to organize, maintain and store files and documents.
  • Ability to write routine reports and correspondence.
  • Ability to speak effectively before groups of customers or employees of the organization.
  • Ability and comfortability working with people of varying socioeconomic backgrounds.
  • Excellent communication skills. Strong networker, convener, relationship builder and communicator with experience collaborating with diverse work teams and stakeholders.
  • Demonstrated ability to handle confidential and sensitive information possessing skills to handle situations in a professional and diplomatic manner.
  • A high degree of collaboration and emotional intelligence, self-motivation, and analytical ability.
  • Proficiency in Microsoft Office Suite and other software routinely used by Heartland Alliance Health.
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
  • Ability to work independently and remain self-motivated with minimal direct supervision.
  • Strategic thought partnership and consultancy skills.
