Integrated Care Coordinator

About the Role:
At Corus Health, we’re reshaping how patients move through the healthcare system. As our Integrated Care Coordinator, you’ll help ensure that every transition — from hospital to home, from skilled nursing to hospice — is safe, timely, and rooted in what matters most: the patient. Based in Albuquerque, NM, this role works across Home Health, Hospice, and Corus Medical to connect care in ways that improve outcomes and reduce avoidable hospitalizations.

Key Responsibilities:

• Act as the central point of coordination across hospitals, skilled nursing, assisted living, and home-based services
   
• Monitor and support timely, appropriate transitions of care
   
• Escalate concerns that may impact patient outcomes, regulatory compliance, or quality metrics
   
• Participate in interdisciplinary huddles, care rounds, and QAPI meetings
   
• Help design and execute Performance Improvement Plans (PIPs)
   
• Track key outcomes such as readmissions, service start timeframes, and satisfaction scores
   
• Use clinical tools to match patients with the right level of care at the right time
   
• Collaborate with the Ensemble Clinically Integrated Network (CIN) and other value-based care initiatives

What We’re Looking For:

• 5+ years in care coordination, nursing, social work, or a related healthcare role (e.g., RN, LPN, SW, CHW, MA, CNA)
   
• Experience with quality improvement, QAPI processes, or PIP development
   
• Solid understanding of Home Health, Hospice, and/or value-based care models
   
• Excellent communication skills and ability to work across clinical teams and systems
   
• Tech-savvy with EMR, spreadsheets, and care tracking tools
   
• Reliable transportation and a valid driver’s license for local travel
   
• A patient-centered mindset, always focused on safe, effective, and compassionate care
