Job Description - Intensive Case Manager

INTENSIVE CASE MANAGER: At OnPoint, the Intensive Case Manager plays a vital role in supporting individuals with mental health challenges, substance use disorders, and/or intellectual and developmental disabilities (IDD) who need a higher level of care and attention. This position maintains a smaller caseload to allow for meaningful, consistent engagement and ensures that each person receives timely, coordinated services that promote stability, independence, and full participation in their communities—whether at work, at home, or in social and recreational settings.
In addition to intensive case management, this role includes providing clinical therapeutic interventions in the community, tailored to the individual’s needs. The ideal candidate holds an advanced degree and appropriate licensure, allowing them to deliver and bill for both case management and therapeutic services. PAY RANGE/BENEFIT PACKAGE: 
Salary Range: starting at $57,942.80 up to $79,961.14 annually - placement above minimum salary is based on experience.

REQUIRED QUALIFICATIONS:
  • Master’s degree in social work, psychology, human services, or a related field.
  • Meet the qualifications of a QMHP or QIDP.
  • Experience working with individuals who have complex behavioral health needs or require intensive case management.
PREFERRED QUALIFICATIONS:
  • Experience in community-based service delivery for individuals with high-acuity mental health or IDD needs.
  • Knowledge of the Person-Centered Planning process.
  • Familiarity with crisis intervention strategies and trauma-informed care.
  • Lived experiences with mental illness/developmental disabilities/ co-occurring disorders/substance use disorders valued.
GENERAL RESPONSIBILITIES:
  • Facilitate the Person-Centered Planning process tailored to individuals with complex needs.
  • Provide Intensive Case Management Services through:
    • More frequent and direct contact with individuals on the caseload.
    • Crisis prevention, intervention, and safety planning.
    • Increased coordination with psychiatric providers, therapists, medical professionals, and community resources.
    • Advocacy and support to ensure access to essential services such as housing, employment, medical care, and benefits.
  • Provide Community-Based Therapy as needed, including:
    • Individualized therapy sessions in home, community, or other natural settings.
    • Crisis de-escalation and intervention using evidence-based therapeutic techniques.
    • Short-term therapeutic interventions to support emotional regulation, coping skills, and treatment plan goals.
    • Collaborate with a wide variety of providers to ensure continuity of care.
  • Develop and strengthen Natural Supports including family, friends, religious organizations, neighbors, teachers, employers, and other community members.
  • Conduct internal and external case collaboration and coordination, gather and exchange relevant information to ensure comprehensive care.
  • Serve as a consultant and resource to individuals and their support systems.
  • Maintain comprehensive documentation of services and supports in compliance with funding, regulatory, accreditation, and best practice standards.
  • Monitor service providers to ensure appropriate implementation of person-centered plans.
  • Participate in clinical case conferences, staff development (in-services, conferences, professional organizations), and agency committees as assigned.
  • Support individuals through advocacy, monitoring, referrals, and linkages with other providers.
  • Perform other duties as assigned.
