The Intensive Group Day Activity Coordinator - Luray is responsible for the daily care, activities, and documentation of individuals in Shen-Paco’s Intensive Group Day Support Program. 

Essential Functions:

1. Provides ongoing daily care of individuals in the Intensive Group Day Support program. Care may include but not limited to; assistance with food preparation, feeding, toileting, using the urinal, transferring, showering, changing briefs, pads, etc., dressing, hygiene skills, walking, transportation on outings. 

2. Provides constant supervision to reduce and/or eliminate any behavior and/or health and safety issues. 

3. Provides input and planning of daily activities for individuals in the program.

4. Provides input and planning and documentation of outings in the community, a monthly activity schedule and participates in outings in the community.

5. Treats all individuals with dignity and respect.

6. Evaluates individuals’ wants and needs; develops, and implements individual activities designed to increase developmental capabilities and functional skills in accordance with Individual Service Plan (ISP).

7. Maintains timely, accurate and complete individual records according to policy and licensure requirements. This includes daily documentation books, progress notes and main files of individuals. 

8. Serve as a team player to support co-workers in promoting SPI mission.

9. Communicates effectively with individual/family/caregiver/case management about any needs pertinent to the individual.

10. Communicates regularly with Intensive Group Day Lead about any important issues dealing with individuals. 

11. Ensures overall safety of individuals and enforces/safeguards individuals’ rights.

12. Willing to fill in other departments, jobs, and work locations as necessary.

Education and Experience:

§ High school diploma or GED. 

§ Prefer some experience working with persons who are developmentally disabled. Ability to work well with a variety of people.

§ Proficient with Microsoft applications.

§ Working knowledge of individual program planning, and individual training methods.

§ Excellent interpersonal, verbal, and written communications skills.

§ Ability to work independently, and within a team environment.

§ Capacity to work effectively and cooperatively with others.

§ Excellent organizational, professional and communication skills.

§ Attention to detail.

§ Must be a good listener.

Licensure and Certification:

§ First Aid / CPR / AED

§ UKERU

§ Direct Support Professional Training

§ Safety Training 

§ Home and Community Based Settings Training

All Positions:

§ Must be 18 years of age.

§ Must possess a valid driver’s license and have proof of a good driving record.

§ Pass multiple background inquiries (both pre-employment and during employment).

§ Pass a drug screen, and then randomly.

§ Must follow SPI vehicle guidelines and policies.

§ May be required to sit or stand for extended periods of time.

§ Willing to work in other departments and locations as needed. 

§ Administers UKERU, First Aid and CPR when necessary. 

§ Follows infectious disease policies and protocols.

§ Must protect the human rights of each individual, ensuring the confidentiality of his /her records.

§ Attend meetings and training courses as required. 

§ Meets attendance standards, being on time and regular in attendance, using PLT wisely.  

§ Must be able to move 50 lbs. and on occasion up to 150 lbs. (lift, push, pull – move objects or individual).

§ Ability to read a variety of documents, evaluations, summaries, reports, charts, reviews, treatment plans etc. Must be able to use a computer, and write documents listed previously, using designated formats with the rules of grammar, punctuation, spelling, dictation, and style.

§ Need to possess the ability to apply the principles of critical thinking, defining problems, emergencies, data collection, stating facts and draw valid conclusions. Must be able to understand both verbal and written forms and instructions.

§ Must be able to communicate and deliver instructions effectively and professionally, both written and verbal, and via telephone.

§ Must have the physical and mental capability to use all office equipment (phone, computer, email, and all functions of a copier).

§ Must be able to handle all types of stressful situations.

§ Must have good personal hygiene, being clean, and neat in appearance, wearing appropriate clothing and shoes for the job.

§ Adheres to the policies and procedures established by the SPI Board of Directors and Management.

§ All other duties as assigned by management.
