Responsible for assisting Campus Director with planning, organizing, developing, and directing the overall operations of the Assisted Living department. Responsible assessing potential Assisted Living residents and current Assisted Living residents’ needs to ensure that the appropriate service package is determined for services. Conduct all duties and responsibilities in accordance with department and Cantata’s policies and procedures, current federal, state and facility standards, guidelines and regulations governing our Company.

Responsible for proactively interacting, as a member of the management team, with the Campus Director and other departments to promote cohesive teamwork that ensures the well-being of the residents and employees.
Responsible for developing, implementing, and maintaining policies and procedures specific to the Assisted Living department. Ensures all workflows align with regulatory standards, promote operational efficiency, and support high-quality resident care.
Collaborates with interdisciplinary teams to evaluate current practices, identify areas for improvement, and establish clear, consistent protocols that enhance staff performance, compliance, and overall service delivery. Responds, in a timely manner, to inquiries and evaluates the initial needs to determine the appropriate service package.
Pre-qualifies admission to the Assisted Living based on the initial evaluation and makes recommendations regarding the service package required for the resident.
Works together with the Campus Director to re-qualify a resident for admission into assisted living following a hospital or other health care setting stay.
Monitors patient progress and communicates information to all appropriate parties.
Participates in and initiates meetings with appropriate staff regarding service delivery and documentation.
Participate in evaluation of day-to-day care of residents adjusting work plan as needed.
Participates actively in a liaison team that helps foster and maintain relationships with hospitals, community agencies, and other referral sources.
Creates and maintains an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the department.
Presents in-services, meetings and educational training in accordance with department and Cantata’s policies and procedures.
Consistently demonstrates an ability to respond to changing situations in a flexible manner.
Supervises staff, which may involve direct and indirect responsibilities for interviewing, selection, training, motivating, wage and salary administration, performance appraisal, developing goals, and developing department policies and procedures to ensure achievement of goals.
Schedule department working hours, personnel, and work assignments, as required and necessary within budgetary guidelines established by the Resident Service Director.
Counseling/disciplining personnel as necessary in accordance with department/company policies and procedures.
Requirements
Knowledge/Skill/Experience
Registered Nurse or Licensed Practical Nurse with current Illinois license.
Must have a minimum of one (2) year experience in providing direct services to the elderly.
Prior experience with the assessment process within an assisted living setting helpful.
Current CPR certification.
Prior staff supervisory experience
Must possess the ability to make independent decisions when circumstances warrant such action.
Must be able to cope with the mental and emotional stress of the position.
Ability and willingness to work harmoniously within a team environment.
Ability to communicate effectively both verbally and in written form with a diverse group of people both inside and outside the organization.
Must possess the ability to deal tactfully with personnel, residents/patients, family members, visitors, government agencies/personnel and the general public.
Must be able to read, write, speak and understand the English language.
Aptitude for detail work.
Basic computer literacy.
Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
Contact with Others
Extensive contact with residents/clients on a daily basis. Daily contact with all levels of personnel within the company. Heavy interaction with client’s family members and community contact
Responsibility for Accuracy
Omission or incorrect mandatory federal, state and facility documentation can generate penalties enforced by regulators.
Responsibility for Confidential Data
Employee-Partners are required to safeguard sensitive information. Sensitive information includes, but shall not be limited to, information concerning Cantata’s former and current residents/clients, medical and health information and Cantata’s information systems including but not limited to the Internet and E-mail.
Physical Requirements/Working Conditions
Must meet general health requirements as set forth by this facility.
Works in residents’ rooms and areas designated for assessments.
May be requested to work beyond normal working hours which may include weekends and holidays when necessary.
May be required to fill in for Nurse “call offs” and other circumstances that create an opening on the schedule.
Is subject to callback during emergency conditions (e.g., severe weather, evacuation, post disaster, etc.).
Sits, stands, bends, lifts, walks and climbs stairs, as needed to conduct assessment activities.
Must be able to push, pull, move, and/or lift a minimum of fifty pounds.
May be necessary to assist in the evacuation of residents during emergency situations.
Exposure Classification
Job duties are classified as Category I (routine exposure to blood, body fluids or tissues).
Routine exposure to hazardous chemicals which are identified for use in nursing department.
