Position Overview:

Our client is a thriving community outpatient medical center seeking a dedicated Internal Medicine Physician to provide comprehensive, patient-centered adult primary care on-site at its Brooklyn practice. The ideal candidate is a personable, energetic physician who thrives in a dynamic, efficient practice, builds lasting relationships with patients, and is excited to help move the practice forward as it grows. This is an in-person outpatient role, with the possibility of some telehealth as part of the schedule.

Responsibilities:

• Provide comprehensive adult primary care on-site in a community outpatient setting
• Manage a broad range of acute and chronic conditions including, but not limited to, diabetes, hypertension, and cardiovascular disease
• Conduct routine examinations and screenings with an emphasis on preventive care and early detection
• Build and grow a loyal patient panel through warm, engaging, and consistent patient relationships
• Coordinate in-house referrals across more than 30 specialty services and ensure continuity of care for patients with complex needs
• Contribute ideas and energy to the practice's growth, including new services, workflows, and patient engagement
• Collaborate within a multidisciplinary clinical team to deliver coordinated, patient-centered care
• Maintain thorough, accurate documentation and adhere to Medicaid, regulatory, and ethical standards

Qualifications:

• Medical degree from an accredited institution (MD or DO)
• Board Certification or Board Eligibility in Internal Medicine
• Valid New York State medical license and DEA registration
• Active New York Medicaid provider number — required
• Strong clinical acumen in chronic disease management and preventive care
• Warm, engaging personality with a genuine passion for serving diverse, inner-city communities
