Night shift IOP/PHP Clinician/Therapist-part time


JOB SUMMARY: The Therapist is responsible for providing quality patient care, displaying skills in social assessments, treatment planning and discharge planning. The Therapist will lead Primary Treatment Modalities, Group, Family Therapies and support the Social Services department.

MAJOR DUTIES AND RESPONSIBILITIES Program Development: The duties of the Therapist shall be in keeping with accepted standards for the state employed and the rules and regulations of the Hospital.

III.DUTIES AND RESPONSIBILITIES:

1. Patient Care (Essential Function): Complete an age specific psychosocial evaluation on assigned patients in a timely fashion.

2. Conduct Group, and Family Therapy sessions focusing on the primary issues of the patient.

3. Engage in timely, efficient discharge planning, assuring the transition of patients to the appropriate level of care as determined by the patient's needs.

4. Assist in the orientation of new patients to the program.

5. Provide for patient comfort, safety, respect and confidentiality.

6. Complete documentation requirements in a timely manner in accordance with program standards.

7. Conduct/coordinate pre-admission screenings as assigned.

8. Ensure health and safety of all patients by observing mental and physical condition of patients on each shift.

9. Perform related duties as assigned.

Professional Responsibilities (Essential Function):

1. Maintain competencies as required by program standards and credentialing organizations.

2. Provide activities for the patients which include daytime, evening, indoor, outdoor, and special and holiday events as evidenced by documentation in the progress notes.

3. Complete admission and discharge forms according to Outpatient policies and procedures.

4. Conduct assessments

a. Make a written report of initial patient assessment, evaluation, treatment and progress as evidenced by initial assessment forms completed in the medical record, and documentation on the progress notes.

5.Request needed supplies for activities:

a. Request supplies and equipment as evidenced by requisitions to the purchasing department.

b. Keep equipment safe for use and in working order as evidenced by work orders for any repairs.

6.Perform other duties and special projects as required.

a. Attend required training and continuing education as evidenced by attendance on training form.

b. Assume additional duties as assigned as evidenced by Management.

c. Complete all competency-based examinations with at least an 80% passing score prior to the end of probationary period and annually thereafter.

7.Follow the policies and procedures of the program.

8. Interact with patient’s support system, e.g. family, resource coordinators, other agencies, etc., to coordinate patient’s treatment.

9. Participate as a member of the multi-disciplinary treatment team attending patients, staffing and treatment team meetings, and provide input in the development of the treatment plan.

10. Attend regularly scheduled staff meetings to receive appropriate supervision and receive Out Patient-specific information.

11. Maintain compliance with staff development training.

12. Routinely demonstrate a sound understanding of and commitment to complying with DNV standards for the IOP/PHP programs.

13. Maintain good attendance, a professional appearance and a positive punctuality history.
Quality Improvement (Important Function):
1. Consistently display cultural competence and sensitivity and ensure services provided(assessment/evaluations, treatment planning, discharge planning), and incorporate patient specific cultural values.
2. Participate in the collection, collation and evaluation of data for outcome studies.
3. Consider/process feedback regarding performance to improve competence.
4. Identify/report variables affecting the quality care provided to patients in a timely manner.
5. Participate in the program quality improvement process and utilization review.

IV.EDUCATION, TRAINING AND EXPERIENCE REQUIRED:

A. Education

1.Master's degree from an accredited school of social work, Clinical Psychology or related field.

B. Training

1.Documented training related to the primary population served within the past year

.2.Eligible and acceptable to meet the expectations of the Hospital.

C. Experience:

At least one (1) year experience in an acute behavioral health setting preferred.

A. Mental and Emotional Demands:

1.Ability to organize, coordinate and evaluate.

2.Ability to cope with anger/fear/hostility of others in a calm manner, manage altercations and confrontations, and assist with problem resolution.

B. Physical Demands:

1.Ability to physically assist patients.

VII.WORKING CONDITIONS:
The job is subject to normal indoor working conditions with adequate lighting and ventilation. Working with patients who have the potential for angry, violent or combative outburst.
