Job Developer - Discharge Specialist

MINIMUM QUALIFICATIONS

Bachelor’s degree with one-year experience, or associate’s degree with 2 years’ experience, or registered/certified counselor with 1-2 years working in substance abuse treatment, recovery or prevention services.

DESCRIPTION

The Discharge Specialist will facilitate and implement care coordination and advocacy for options and services to meet an individual’s and family’s comprehensive life skills plan through communication and available resources to promote healthy effective outcomes, for men or women in recovery from substance abuse. Implement discharge plans.

NOTE: Employees will be required to be fingerprinted and will be subject to clearance by the department of Justice.  TB Testing is a requirement.
