Overview:
We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Patient Care Manager in the Home Health sector located in Ormond Beach, FL. In this role, you will ensure high-quality patient care, lead a team of healthcare professionals, and oversee compliance with clinical and regulatory standards.

Key Responsibilities:

• Oversee the development, implementation, and evaluation of individualized patient care plans.
• Coordinate and manage patient care activities, ensuring compliance with clinical policies.
• Supervise, guide, and support nursing staff and other healthcare team members.
• Conduct patient assessments and maintain accurate documentation.
• Collaborate with physicians, therapists, and other providers to optimize patient outcomes.
• Ensure compliance with state/federal regulations and accreditation standards.
• Educate patients and families on care plans and health maintenance.

Requirements:
Education:

• Current Registered Nurse (RN) license in Florida.
• Bachelor’s degree in Nursing (BSN) preferred.

Experiences:

• Minimum 2 years of clinical nursing experience, with at least 1 year in home health or community health.
• Home Health leadership experience required.
• 2 years of management/supervisory experience (preferred).

Other:

• Strong organizational, communication, and interpersonal skills.
• Proficiency in electronic health records (EHR) and computer applications.

Compensation and Benefits:

• Salary: $80,000 - $85,000 per year
