Ash & Harris Executive Search is looking for a Physical Therapist

Overview:

We are seeking an experienced Physical Therapist to join our team, providing high-quality patient care in the comfort of their homes. The ideal candidate will evaluate and treat patients under physician orders, utilizing therapeutic procedures to restore strength, coordination, and mobility while relieving pain. This role involves collaboration with an interdisciplinary team to ensure comprehensive patient care.

Key Responsibilities:

• Conduct physical therapy assessments, diagnostic tests, and skilled procedures.
• Develop and update individualized therapy care plans.
• Document findings, interventions, and outcomes in compliance with agency policies.
• Communicate care plans and changes to physicians, case managers, and caregivers.
• Implement patient and caregiver education on physical therapy needs and symptom management.
• Participate in interdisciplinary team meetings and discharge planning.
• Ensure compliance with safety, infection control, and regulatory standards (OASIS, CHAP, JCAHO).
• Supervise home health aides and physical therapy assistants as needed.
• Contribute to quality improvement initiatives and clinical record reviews.

Requirements:

Education:

• Graduate of an accredited Physical Therapy program (APTA, CAHEA, or AMA-approved).
• Current state licensure/registration as a Physical Therapist.
• Current CPR certification.

Experience:

• Minimum 2 years of healthcare experience.
• Home care experience preferred.

Other:

• Strong verbal and written communication skills.
• Ability to work independently with minimal supervision.
• Valid driver’s license, insured vehicle, and clean driving record.

Compensation and Benefits:

• Salary: $150,000 - $175,000 per year
