Job Description - (Job TE- 1089) Elder Care Advocate

Ash & Harris Executive Search is looking for an Elder Care Advocate

Overview:
Our client, a leading Long Island-based law firm specializing in Elder Care and Estate Planning, is seeking a compassionate and knowledgeable Elder Care Advocate to join their Care Coordination Team. This role is vital in helping aging individuals and their families navigate the Community Medicaid Homecare process, ensuring they receive the necessary support to remain safely at home.

Key Responsibilities:

  • Conduct in-home and virtual assessments to determine eligibility for Community Medicaid home care services.
  • Collaborate with attorneys, paralegals, and social workers to create personalized care plans.
  • Guide clients through MLTC (Managed Long-Term Care) enrollment calls and refer them to waiver programs as needed.
  • Educate clients and families on local aging resources and services.
  • Coordinate with outside agencies to meet clients’ ongoing needs.
  • Build relationships with home health-care agencies to facilitate staffing of Medicaid-funded aides.
  • Travel regularly across Nassau and Suffolk Counties for home visits.

Requirements:
Education:

  • Bachelor’s degree in Social Work, Gerontology, Healthcare Administration, or a related field (preferred).

Experience:

  • Familiarity with Community Medicaid, CDPAP, and PCA services.
  • Knowledge of short-term and long-term care planning.
  • Understanding of Assisted Living Facilities (ALF), Assisted Living Programs (ALP), and senior living options.
  • Experience navigating senior services in Long Island.

Other:

  • Strong communication and interpersonal skills.
  • Compassionate and client-focused approach.
  • Ability to travel locally for home visits.

Compensation and Benefits:
Salary: $55,000 - $65,000 (commensurate with experience).

  • Mileage reimbursement for travel.
