@Southwest Louisiana Independence Center (@SLIC) is dedicated to empowering individuals with developmental disabilities and the elderly to live with independence, dignity, and full participation in their community.

We are seeking a full-time Case Manager to join our team. The Case Manager will play a key role in connecting individuals and families with services, resources, and support tailored to their unique needs.

Responsibilities of a Case Manager:

• Monitor home and community-based services through regular monthly phone calls and quarterly home visits (with additional visits as needed based on the individual’s plan of care).


• Maintain accurate documentation of changes, updates, or additional needs to ensure goals in the care plan are met.


• Address and resolve any concerns related to services, scheduling, or staffing.


• Recruit, hire, and train qualified Direct Service Workers (DSWs) to provide personal in-home care.


• Ensure DSWs are supporting clients in achieving the goals outlined in their plan of care.


• Conduct unannounced visits to observe and evaluate DSW performance.


• Participate in an after-hours on-call rotation to provide support as needed

Job Requirements:

• Minimum of 2 years of experience working with elderly and/or disabled individuals


• Ability to work independently with minimal supervision


• Strong organizational and time-management skills


• Knowledge of local community resources and support services


• Excellent written and verbal communication skills


• Familiarity with waiver programs such as New Opportunities Waiver, Children’s Choice Waiver, Community Choice Waiver, Supports Waiver, EPSDT, and Long-Term Personal Care Services is preferred but not required


• Salary is commensurate with experience
