Personal Service Coordinator / LMFT / MFT / LCSW / ASW / APCC / LPCC

35 hours per week / $34.00 - $39.00 per hour

(The actual compensation will be determined based on experience and other factors permitted by law.)

Summary: MHALA is partnering with Dignity Health, which will provide targeted outreach and case management services to individuals who frequently visit the St. Mary’s Hospital Emergency Room, and who also may be experiencing challenges such as mental illness, homelessness, substance use, or difficulties accessing healthcare. The Clinical Personal Service Coordinator on this team in general will be responsible for completing assessments of individuals referred to the HealthLink program by the St. Mary’s Emergency Room staff, and developing an appropriate recovery plan to connect individuals referred to needed services utilizing practices of harm reduction, housing first, and  motivational interviewing.

Essential Functions, Duties and Responsibilities:  This is not an exhaustive task list, and other duties may be assigned as deemed appropriate or necessary by the supervisor or program director.   • Responsible for outreaching and assessing individuals referred to the HealthLink program by Emergency Department staff.
  • Perform thorough biopsychosocial assessments and develop individual recovery plans as deemed appropriate by assessment, including but not limited to linkage to emergency/transitional housing, mental health services, insurance, healthcare providers, benefits, substance abuse treatment, and any other needed resource.
  • Utilize evidence based practices in service delivery such as motivational interviewing, harm reduction, intensive case management, housing first, and critical time intervention
  • Provide brief solution-focused clinical services as needed including brief therapy/counseling and crisis intervention.
  • Provide community-based case management services, and facilitate warm hand-offs of individuals from the HealthLink program to longer term supports.
  • Develop strong working relationships with local programs and resources to facilitate linkage of individuals as needed.
  • Work collaboratively with St. Mary’s staff to facilitate referrals to HealthLink program, participate in collaborative meetings with St. Mary’s, and provide active feedback on development of program.
  • Provide timely information and feedback to supervisor with regard to emergent participant and program issues.
  • Oversee and advocate on behalf of the member to help him/her obtain and keep housing, health care benefits, entitlements, etc.
  • Attend and participate in staff meetings and other aspects of the day-to-day operations and community life of the program site(s)
  • Complete all documentation and maintain all paperwork and other required documentation as required for program compliance and accountability in accordance with MHA standards and funding source requirements.

Other Position Requirements:

  • Licensed Clinical Social Worker or Associate Clinical Social Worker, Licensed Marriage and Family Therapist or Marriage and Family Therapist Intern, Licensed Professional Clinical Counselor or Associate Professional Clinical Counselor, or Psychologist.
  • Bilingual; Spanish/English speaking preferred but not required.
  • Familiarity with local community resources for individuals experiencing homelessness, mental illness, and/or low income
  • Must have good basic English writing skills, good basic computer skills and be highly organized. Proficient in computer applications such as Microsoft Word, Excel and Outlook.
  • Must have excellent communication skills and be able to multi-task.
  • Must function well in a team-oriented and fast-paced environment to member services
  • Driving with current class C driver’s license, a vehicle for transportation, proof of valid automobile insurance, the ability to be insured by MHALA’s liability insurance are conditions of employment
  • TB clearance and re-testing every year are conditions of continued employment
  • Employment verification directly from three (3) past employers within the most recent 7-year period
