Job Description - LCSW - Behavioral Health Program

Compensation: $84K-95K per year (Negotiable)

About Us:

Join a well-known and highly respected nonprofit community health agency in San Jose. The IBH Counselor assesses and treats patients in the Health Center, acting as a bridge between physical and behavioral health. This role is responsible for offering culturally sensitive short-term therapy to referred patients, working in collaboration with Health Center providers, and integrating care to promote the overall well-being of each patient. At a minimum, the Counselor dedicates at least 50% of their time to providing direct billable services.

Responsibilities:

  • Provide culturally sensitive, client-centered individual, group, and family counseling and rehabilitation services.
  • Facilitate referrals to community resources and refer to case management services.
  • Conduct psychosocial assessments to identify patients' needs and formulate treatment plans.
  • Document and maintain clinical records per appropriate standards and regulations.
  • Participate in clinical supervision, team meetings, and trainings.

Requirements:

  • Licensed LCSW in the state of California.
  • Master’s degree in social work or psychology.
