ORGANIZATIONAL BACKGROUND:
The Central California Asthma Collaborative (CCAC) is a non-profit organization dedicated to reducing the burden of asthma and other health disparities in the San Joaquin Valley.
POSITION SUMMARY: Under the direction of the Site Supervisor, the LCM 2 will work with health plans, healthcare providers and other CCAC contracted entities, to implement CCAC’s Enhanced Care Management (ECM) services and other related health supportive programs and projects, which includes but may not be limited to, home visits, creating a care plan that meets the requirements of the health plans and addresses identified barriers. The LCM 2 shall act as a point of contact and coordinator for the client's providers including Primary Care Provider(s), Specialists, Housing Service Providers, SUD Service Provides, and other CALAIM Community Support providers as prescribed by the care plan. The LCM is an integral part of the CCAC team working toward achieving health equity in the San Joaquin Valley.
Primary Responsibilities
· Schedule and conduct ECM visits including patient health assessment, needs assessments, and other assessment tools as needed.
· Track and schedule follow-up appointments with clients. Maintain confidential records containing client information.
· Conducts regular reviews of patient’s chart to ensure quality services are provided and documented by Lead Care Managers consistently and accurately.
· Work with the Clinical Consultant to recommend and coordinate interventions and referrals, reporting back on their success. · Coordinate client’s additional providers and services to ensure all of client’s needs are met.
· Coach clients in learning and developing daily living skills needed to improve and sustain independent living.
· In a timely manner, document assessment and client information using appropriate report forms and/or electronic health record in accordance with office standards and regulations.
· Assist the leadership team in preparing and submitting monthly, quarterly, and/or six-month progress reports to various contractors and others as required.
· Bridge community, cultural, linguistic, socioeconomic, and educational barriers to encourage self-care and participation in the program.
· Attend on-going CCAC in-service trainings.
· Perform all other CCAC duties as assigned.

DRIVING AND LICENSE REQUIREMENTS:
Must possess a valid CADL and applicable CA mandated auto insurance
PHYSICAL REQUIREMENTS:
Lift up to 20 (twenty) pounds.
AUTHORITY AND SUPERVISORY CAPACITY: In limited situations, may supervise volunteers and/or interns.
ACCESS TO SENSITIVE INFORMATION: Must maintain confidentiality of personal client and staff information exposed to when working in files and preparing reports.
