The Lead Case Manager will provide direct client care while also supporting the operational oversight of the case management team. Primary responsibilities include facilitating in-person psychoeducational groups, carrying a designated caseload, documenting group, individual and completing individual service/treatment plans for clients receiving services.
In addition to direct service delivery, the Lead Case Manager will provide guidance and support to case management staff, assist with program coordination, ensure documentation compliance, and support the Clinical Director with daily program operations.
The Lead Case Manager will also perform outreach calls, and coordinate case management services for individuals experiencing mental health and co-occurring disorders.
Eligible candidates must have experience working in the behavioral health field, demonstrate strong leadership and organizational skills, and possess a commitment to providing quality services and support to individuals in the community.
Essential Duties and Responsibilities
Facilitate psychoeducational and therapeutic groups for program participants
Maintain and manage an assigned client caseload
Develop, implement, and update individualized treatment/service plans
Document group, individual, and family progress notes in accordance with program standards
Coordinate case management services, including referrals to community resources
Conduct outreach calls and engagement activities to support client retention
Provide guidance and mentorship to case management staff
Assist with staff training and onboarding of new case managers
Support the Clinical Director with program coordination and daily operations
Monitor documentation quality and timeliness to ensure regulatory compliance
Participate in multidisciplinary treatment team meetings
Collaborate with community providers, probation/parole officers, and external agencies
Assist with maintaining Joint Commission and regulatory compliance standards
Support crisis intervention and client support as needed
Required Skills and Abilities
Ability to work as a team leader and collaborative team member
Strong management, communication, organizational, and interpersonal skills
Ability to work under stressful conditions and remain flexible to departmental needs
Education and Experience
Bachelor’s degree in a related field preferred
Associate’s degree with two (2) years of relevant experience will be considered
High School Diploma or GED with five (5) years of relevant experience will be considered
Preferred experience includes:
Experience working with individuals with behavioral health conditions (trauma, mental illness, and/or substance use disorders)
Experience providing case management services in the mental health or substance use treatment field
Experience facilitating psychoeducational or therapeutic groups
Experience working within a multidisciplinary treatment team
Knowledge of the criminal justice system and community resources
Computer literacy, including proficiency with Microsoft Office applications
