How You'll Make a Difference:

The position of Lead Care Service reports directly to the Transitional and Post-Acute Teams Manager and ensures adherence to all standards and expectations of the Care Service Coordinators (CSC). The Lead Care Services Coordinator will be responsible for facilitating care for patients by coordinating and scheduling with providers. Care Services Coordinators support the patients through timely, high quality, and efficient health care. Our care includes all transitional/post-acute support services through the comprehensive adherence to the multi-disciplinary care plan, patient self-management goals and referrals to both internal and external resources.

Essential Duties and Responsibilities:
Provide guidance through clinical leadership to the CSC
Assist and resolve any issues with patients, health plan, providers, or facilities in a professional and timely manner
Identify and submit to the Transitional and Post-Acute Teams Manager any potential quality issues for review
Prepare and investigate any quality issues or complaints involving the CSC's
Conduct weekly audits of CSCs work to ensure CMS corporate and partner compliance is met
Prepare and maintain weekly and monthly CSC metrics
Participate and equally share in after-hour and week-end coverage for the Clinical Services Department

Qualifications/Skills:
High school diploma or GED required
MA, CNA or former medical office experience preferred
Some college or medical certification preferred
Knowledge of medications, medical terminology and patient care preferred
Bilingual is a huge plus
Weekend availability is not required, but is a plus
Willing to maintain ongoing education as requested to support position
