Responsibilities

• Provide direct nursing care to patients in their homes, following established care plans and physician instructions.


• Monitor patients’ health conditions and report any changes or concerns to the supervisory team.


• Administer medications and treatments as prescribed.


• Maintain accurate and detailed patient records.


• Communicate effectively with patients, families, and healthcare professionals to ensure coordinated care.


• Adhere to healthcare regulations, policies, and procedures to maintain a safe and ethical work environment.

Requirements

• Valid Licensed Practical Nurse (LPN) license.


• Strong interpersonal and communication skills.
