Elk Valley Health Services is seeking compassionate and reliable Licensed Practical Nurses (LPNs) to join our Private Duty Care team. In this role, you will provide one-on-one nursing care to patients with complex medical needs in the comfort of their homes. Responsibilities

• Deliver high-quality clinical care within the scope of nursing practice as defined by Tennessee state law.
• Follow the patient’s plan of care and coordinate services with physicians, family members, and other healthcare professionals.
• Maintain accurate, timely, and thorough documentation in the electronic medical record system, including electronic visit verification when required.
• Actively participate in care coordination by communicating patient needs, changes in condition, and updates with the care team.
• Adhere to infection control standards and maintain strict patient confidentiality at all times.

Requirements

• Current Licensed Practical Nurse (LPN) license in the state of Tennessee.
• Current CPR certification.
• Valid driver’s license, reliable transportation, and proof of insurance.
• Previous home health or private duty nursing experience preferred, but not required.
