We are seeking a licensed clinician with a heart for patient advocacy and a passion for building professional relationships to join our growing team.

Key Responsibilities

• Build and grow relationships with hospitals, rehab centers, the VA and assisted living facilities.


• Collaborate with hospital and rehab discharge planners, case managers and physicians to identify patients suitable for home care.


• Provide education to patients and families about our home care services.


• Conduct clinical assessments to determine eligibility and service needs


• Represent Senior Helpers within the local senior community at events and programs.

Qualifications

• License as an RN, LPN, or LCSW (preferred)


• Experience in discharge planning or case management strongly preferred


• Strong interpersonal and communication skills


• Highly organized, self-motivated, and goal-oriented


• Valid driver’s license and reliable transportation

Compensation

•  Total Compensation: $65,000 – $110,000 (depending on license and experience)

• Bonus Structure: Performance-based bonuses tied to referrals and admissions
